"Ingresa a la unidad, alerta, colaborador, en ritmo sinusal ... se solicitan paraclínicos de control, *hoja neurológica*, CSV [control de signos vitales], atentos a evolución clínica..."

What is this hoja neurológica, and what would we call it in English?
