En el proyecto de salud en coordinación con el Departamento de Salud y liderazgo comunitario se fortalecieron capacidades locales de 22 brigadistas de salud que dan seguimiento en las comunidades como facilitadores de espacios de reflexión acerca de la importancia de las buenas prácticas de salud, jornadas de higiene, lo que tiene como fin la prevención de enfermedades en niños.

My try:
The Health project in coordination with the Deparment of Health and with community leaders managed to build local capacities of 22 health promoters who follow up communities in their capacity of facilitators of spaces to reflect on the importance of good health practices, hygiene campaigns which purpose is preventing childhood illnesses.

rephrase the whole sentence
Explanation:
Many texts in the “development” field are erudite – almost to the point of being meaninglessness. It helps to cut through the verbiage and discover what is really happening ‘on the ground’. It does no harm in these circumstances to get away from the terms of the ST and explain, rather than just translating.

The ST is simply describing an instance of the classic “training the trainers” principle*, which has been at the heart of development in the emerging economies (ex 'third world') since well before I was a volunteer worker (in Central America) in 1975. It will help the ST author’s cause if the translation can get past the erudite “medium” and instead clearly transmit the message.

For example (for the entire sentence):

The Health project, in conjunction with the Department of Health and community leaders, strengthened the skills of 22 local health promoters. These people are now passing on their skills to the communities through the organization of hygiene sessions where local people can become (or ‘be made’) more aware of the importance of good health practices, all with a view to preventing childhood illnesses.

I think your idea is spot on with the literal “spaces to reflect on”, however, it’s a rather long sentence, so I think you need to try to make it seem a little less long somehow in English.

Also, a lot depends on how you translate “dan seguimiento en”.

Suggestion:

“En el proyecto de salud en coordinación con el Departamento de Salud y liderazgo comunitario se fortalecieron capacidades locales de 22 brigadistas de salud que dan seguimiento en las comunidades como facilitadores de espacios de reflexión acerca de la importancia de las buenas prácticas de salud, jornadas de higiene, lo que tiene como fin la prevención de enfermedades en niños.”

“In coordination with the Department of Health and community leaders, the health project has strengthened the local capacities of 22 health promoters who provide ongoing support to communities as facilitators of spaces to reflect on the importance of good health practices, as well as hygiene campaigns, for the purposes of childhood disease prevention.”
