Like a paystub (from years gone by), but here it's so guardians/parents can acknowlege they've seen their child's grades.

Though wordier translations may be possible and 'talón' probably refers to a perforated, detachable stub or (bottom portion of a report card), perhaps 'acknowlegement' would suffice:

"The final day to return the acknowlegement is in 5 days."

Or a less literal option
