You may use clinic or some other wording, but patient could be discharged with follow up instructions (without any appointments as an outpatient) or with follow up by a GP. Here it states that their own outpatient service will be in charge of patient's follow up -and not just any outpatient service or any cardiology outpatient service, but the outpatient clinic of the cardiology service responsible of patient's hospital discharge.

Phil may be right anyway; you might say "with follow up appointments at our own (cardiology) clinic" or something along this line.

And anyhow, consultas means consultas externas (outpatient clinic) and propio servicio means our own, the same service responsible of patient's discharge, just as Reuben has already explained.

consultas del propio servicio = visits in its own service, in the same department; meaning the same department from which the patient was discharged.

Follow-up can take place with a family doctor after seeing a specialist. This is not the case here, it's as an outpatient, and in English the formulation "in the same service/department" isn't used; "as an outpatient/at the outpatient clinic" is.

The Asker had a doubt if "propio" was "with own doctor" or "suitable/relevant". It isn't, it's "the same department from which the patient was discharged".

But follow-up would be done on an outpatient basis, so it goes without saying. The French doesn't say "ambulatoire".

Here, "outpatient" is necessary.

"Motivo de Alta" means "reason for discharge" = the hospitalized patient is going home, and will have follow-up with the cardiologist at the hospital as an outpatient, at the "outpatient clinic" [in the same department/service from which they have been discharged].
