concentration index Explanation: Denní odpad vody a jednotlivých iontů Stanovení množství tekutiny a iontů vyloučených v definitivní moči za den je důežité pro diferenciální diagnózu. Vyžaduje 24hodinový sběr moči. Frakční exkrece (exkreční frakce, FE), tubulární resorpce (TR) a koncentrační index Frakční exkrece (FE) je poměrná část, která se z množství profiltrovaného v glomerulech nakonec vyloučí v definitivní moči. Určuje se měřením v jednorázově odebraném vzorku jako poměr clearance vyšetřovaného iontu, solutu anebo vody ke clearance kreatininu; vzhledem k vykrácení poměrů objem/čas není nutný sběr moči. Pro frakční exkreci a tubulární resorpci platí vztah: TR = 1 – FE. Od veličin FE a TR je třeba odlišovat tzv. koncentrační index.- poměr hodnot v moči a v plazmě. Např. koncentrační index pro kreatinin se vypočítá jako Ukreat/Pkreat, pro ureu UU/PU, pro osmoticky aktivní látky jako UOsm/POsm atd. Thus, the concentration index, CU/CP, is large for creatinine. Multiplying this index with the urine flow yields a result greater than similar results derived for most other substances (Eq. 25-1). Brandt Rehberg used this concept (later termed clearance) as his measure of renal filtration rate. The work with these matters developed into the idea of a filtration-reabsorption type of kidney. Rehberg was the first to realise that the reabsorption in the proximal tubules controls the filtration. A few years later Rehberg´s renal filtration rate was called creatinine clearance and used as a measure of the glomerular filtration rate (GFR). |
