Dutch term or phrase: PraktijkOndersteuner Huisartsenzorg (POH)
I know that PraktijkOndersteuner can be translated as nurse practitioner (this term was discussed on Proz in 2008 and the suggestion was doctor's assistant, but that is used elsewhere in my document).
I would appreciate any suggestions of how to translate PraktijkOndersteuner Huisartsenzorg (POH). Is there an official title in English for this or is it just a nurse practitioner in a GP practice/clinic?

General practice-based nurse specialist
Explanation:
General practice-based nursing/nurse specialist

you also find a great number of references for:

'nurse specialist in primary care'

een doktersassistent(e) = a general practice nurse / practice nurse (see below)

a "praktijkondersteuner" especially focuses on chronic illnesses (see also last reference in reference entry)

instead of 'nursing specialist' you can also use 'nurse specialist'
as far as I can see, these are used as synomyms

General-practice-based nurse specialists-taking a lead in improving the care of people with epilepsy.

Epilepsy is almost as common as diabetes and some 750 people with epilepsy die suddenly and prematurely each year. Unfortunately, the management of epilepsy has been much neglected and services often remain fragmented and difficult for patients to understand. We employed a nurse specialist in epilepsy to work with practice nurses in a group of general practices to promote better care, to make patients aware of sources of help and support, and to provide information about issues such as driving, employment and pregnancy.

Introduction

Most people with chronic conditions such as diabetes, congestive heart failure, asthma, and depression are managed in primary care. NHS consultants have traditionally confined their role to patients who are referred to outpatient clinics by their general practitioners. Such patients usually have the most severe and complex problems. Effective care teams for chronic illness must be able to cross practice or organisational boundaries,1 but the current organisational structure of the NHS does not provide many incentives to develop such linkages. General practitioners refer patients they cannot manage and hospitals are funded on the basis of referrals. Time spent on joint work with primary care is not accounted for. To ensure that all patients get the best treatment, the role of consultants needs to change so that their specialist knowledge is more available to everyone dealing with chronic disease.

Changing roles

In recent years nurses in the NHS have taken on a larger role in managing patients with chronic disease. ** Specialist nurses work alongside both general practitioners ** and consultants to assist in managing people with complex problems in both hospital and community settings, and ** practice nurses now act as case managers to patients with conditions such as diabetes in primary care **. However, there has been little integration of primary and specialist care through shared information systems and clinical protocols, such as that seen in US health maintenance organisations.

Reference: nursing specialist

Reference information:

Professional field of nursing specialist

The Advanced Nursing Practice Master diploma allows nurses to get a position as a nursing specialist that traditionally is not part of the professional opportunities of nurses. Nursing specialists will be authorized, as so called article 14 professionals under the Individual Health Care Professions Act (Wet BIG), to perform certain reserved actions, such as prescribing drugs. One can start working for instance in university medical centres, in general hospitals, in general practitioning, in nursing homes, and in institutions for public health care, mental disability care or mental health care institutions. These nursing specialists, thus diagnose and treat patients with a large variety of health problems.

'superverpleegkundigen'

HBO Advanced Nurse Practitioner & Fysiotherapie
