Mantelzorg is vrijwillige zorg, meestal verleend door een naaste, vaak uit liefde en genegenheid. Hoe goed de bedoelingen ook zijn, als mantelzorgers de zorg voor hun naaste niet meer aan kunnen, lopen ze het risico om te ontsporen in gedrag dat ze zelf liever niet zouden willen vertonen.

Mantelzorg wordt vaak in een adem genoemd met care in the community. Maar ik kom ook "informal care" tegen. Ik zoek nog even naar een paar voorbeelden voor je.

Caring for a relative or close friend with a serious psychological disorder is often a
heavy burden for the care giver and for his or her environment. A study of the Dutch
Expertcenter on Informal care (Expertisecentrum Mantelzorg) reveals that many
people with psychological disorders show changes in personality, are easily irritated
and lack insight into their illness. Many care givers, therefore, need respite care, but
a large number of respite care facilities are not suited for this specific client group of
mental health clients. The study indicates that care farms are an exception. The care
farm is a small-scale, accessible facility that offers appropriate day occupation for
many mental health clients. Thus, the care farm can offer an important contribution
to respite of care givers.

JurLex komt ook met "Informal Care" of "Mutual Care". Dus is Informal care misschien beter als care in the community. "Informal care gone wrong" zou dus een oplossing zijn.

Het wordt uit de context niet duidelijk waar de term 'ontspoorde mantelzorg' voorkomt, maar als je alleen die term vertaald wilt hebben zou je zoiets moeten zeggen als 'job burnout in community care'.

Caregivers are being relied upon in more and more cases, and they are experiencing increasing pressure. Many are overburdened, and this can lead to derailed informal care, or even abuse arising from frustration. An overburdened caregiver has major implications for the client/care recipient. It is therefore important to consider how to reduce the burden on the caregiver.

Vormen van ontspoorde mantelzorg

Ontspoorde mantelzorg kan zich op zes verschillende manieren uiten:

Psychisch: bijvoorbeeld schelden door overbelasting. Dit is de meest voorkomende vorm.
Lichamelijk: bijvoorbeeld het verkeerd toedienen van medicijnen uit onwetendheid.
Verwaarlozing: zoals niet de juiste voeding geven.
Schending van rechten: de zorgvrager niet meer betrekken bij besluiten over zijn of haar leven.
Financieel: zoals het toe-eigenen van spullen zonder instemming van de betrokkenen.
Seksuele grensoverschrijding: als een mantelzorger onbedoeld over de seksuele grenzen heen gaat, bijvoorbeeld door miscommunicatie.

Situaties van ontspoorde mantelzorg beslaan doorgaans verschillende van de hierboven beschreven vormen.

Wat is het verschil met ouderenmishandeling?

Bij ouderenmishandeling wordt een oudere meestal door een familie- of gezinslid en soms een beroepskracht mishandeld. Er zijn drie verschillen met ontspoorde mantelzorg. Ten eerste gaat ontspoorde mantelzorg over alle leeftijdscategorieën, niet enkel ouderen. Ten tweede gaat het bij ontspoorde mantelzorg om de relatie tussen de mantelzorger en de zorgvrager. Dit is in een deel van de gevallen van ouderenmishandeling ook het geval, maar het kan tevens om een zorgvrager en beroepskracht gaan. Tot slot is een kenmerk van ontspoorde mantelzorg dat opzet ontbreekt terwijl het bij ouderenmishandeling om opzettelijke mishandeling én ontspoorde mantelzorg kan gaan.

According to the Dutch expertise centre for informal caregiving, overload can manifest itself in the following ways (Expertisecentrum Mantelzorg, 2009). Stress responses are the first symptoms of overload. These include poor sleeping, reduced appetite, nervousness and irritability. Some people seek to reduce stress by taking addictive substances such as sedatives, alcohol and drugs. At this stage, informal caregivers experience these symptoms as natural rather than odd, which means they tend not to visit their doctor. Another reason is that they are so busy with their other activities. Daily caregiving often causes physical symptoms, especially back problems. Performing household tasks can also lead to physical symptoms. These symptoms may get worse if the informal caregiver has a disturbed night’s sleep (e.g. from helping the care recipient go to the toilet).

Symptoms of overload can become aggravated if the care burden increases or the care need is of lengthy duration (Alice Boer, Groenou, & Keuzenkamp, 2011). It may
even result in burnout. The first stage is marked by physical and emotional exhaustion, which is accompanied in the second stage by a negative, cynical attitude. By stage three, the caregiver has developed a complete aversion to the situation. The last two stages in particular often result in ‘derailed care’, whereby the care recipient is no longer well cared for and may even be abused (Royers, 2007).
