Do these 'activeringsprogramma’s' involve getting these people (back to) work, back into society, etc.? I have seen use of the terms 'labour market activation', 'employment activation', 'social activation programme' and just plain 'activation' here and there online, in UK and EU sources. However, this seems like something used in government contexts to refer to policy. Not sure if it is also used to mean getting addicts and ex addicts back to work.

You might want to take a look at this website, about the rehabilitation of homeless veterans, which " involves roughly 90 days of personal problem solving and life-skills training in a peer-support format, and an additional 90 days for job search and employment" in order to "help Veterans return to employment and stable housing while maintaining a clean and sober lifestyle." Even though there is mention of this clean and sober lifestyle, veterans only have to be (at risk of being) homeless (and not addicted to anything) to be eligible for treatment here.

Judging from the context you are giving, would rehabilitation fit the bill? I am not entering it as an answer because, although it would apply to addicts, it would not apply to the homeless non-addicted.
