a script is in form of dialogue between doctor and patient for the purpose of orderly follow-up and tracking of the conditions of a patient. It records the interactive talks between doctor and patient or enable the care team to know what to ask or say to a patient in subsequent follow-up, otherwise, it'll be messed up.

I understand what a "script", but it's very unspeakable here, the best translation I can come up with now is "问诊记录"。
