我原來工作過得那個診所﹐就是四十年前在聯邦的資助下成立的﹐當時Bureau of Primary Care出臺了一項政策﹐如果說願意成立的非營利性的社區診所﹐聯邦可提供資助。大約在2000年左右﹐年資助額度是400萬美元﹐給你資助的同時發給經營者一本書﹐讓經營者必須遵守書裡的規定﹐條文很詳細﹐例如﹐強制性地要求這類診所對低收入者按照收入水平施行分級收費﹐虧空由聯邦的資助給予補充。又例如﹐這類診所必須開展一些不賺錢的項目﹐比如社區健康教育等。也許這就是REGULATED的含義。

紐約市和紐約州出臺的關於公立醫院的規定﹐這個診所沒有義務遵守﹐雖然他們經常選擇參照執行﹐因為是在人家地盤上做買賣。說明這類診所/醫院與州立市立醫療機構是不同的﹐不受當地政府的REGULATE。
