@ PLR Having given the phrase an extra thought, it must be that the embolus is causing the infarction during the operation, it is a complicance that is well known, and that it should be verified with either autopsy or clinical investigation. As Francois says.

I´d say that in both cases the embolus HAS TO BE documented through surgery, autopsy or clinical investigation.
In the first case a definition of "peripheral embolic event" in this particular circumstances is given and in the second case is stated an exclusion criteria, i.e. that patients who have had an infarction are excluded UNLESS it can be proven throught operation, autopsy or clinical investigation that their infarction was caused by an embolus.

I am sorry, but my French knowledge is not sufficient to give the wording in French, I can just give you the meaning!

It means that it has to be proven, either by operation (if the patient is still alive), autopsy (if the patient is dead) or investigational research, that the infarction was caused by an embolus. Does it make sense?
