The header, wrtiiten in lc /lower casing, is slightly ambiguous as could mean a general type and not physical form of affidavit (now in partiuclar contexts like schdules of exhibits, in E&W known as a 'Statement of Truth).

Hier bin ich mir nicht sicher, ob es sich dabei tatsächlich um eine konkretes Formblatt, vielmehr um eine allgemein gefasste Verischerung an Eides statt handelt.

neutral  Katja Schoone: Hatte ich schon vorgeschlagen, funktioniert aber nicht wirklich mit "make oaths", denn bei einer EV oder Erklärung, deklariert man an Eides Statt, also ohne "oath"
