I translated it as

יש לך כאב או אי-נוחיות במידה בינונית?
Or
יש לך כאב או אי-נוחות במידה בינונית

The client says:

When I ran the wording for the pain / discomfort dimension in the telephone version through Google translator some rather strange wording came back, e.g. ‘discomfort’ was translated as ‘non-convenience’!

אי-נוחות
has two translations, in the back translation:

discomfort, inconvenience

The client thinks that Google translation knows better.
