I'm a student at the University of Massachusetts Amherst and as part of an assignment I am translating the client demographic form for the Center for Women and Community on campus. The Center provides women with resources and support (including counseling, leadership seminars, and sexual assault services) and acts as a safe space on campus. The audience for this client demographic form is anyone who wants to use the Center's services. Someone who comes into the Center for whatever reason will first be given this form to fill out before meeting with anyone. The purpose of the form is to get background information on the client so that the Center can serve them better. The form asks the client their name, age, ethnicity, gender identity and sexual orientation. It also asks about any disabilities they may have. In the "Disabilities" section of the form, the client is provided with a list of disabilities and is asked to check off any that they may have. The disabilities listed are: developmental, hearing, learning, mental health, mobility, addiction, and visual. Some of them (like addiction) don't really make sense under "Disabilities" and I have already asked the director of the Center about this; she is working on rewriting the form, but my translation is due before this. I have translated "Disabilities" as "Deficiências" but every native speaker I have asked to review my translation says that this is not correct. However, neither I nor they have been able to come up with a suitable substitute. Are there any suggestions? Thank you very much!

Digo isso como pai de duas crianças com deficiência. Na legislação, é essa a palavra usada.
Inclusive fizemos imposto de renda há pouco tempo, e um dos campos é justamente este - se há alguma pessoa com deficiência na família. O termo usado é realmente deficiência.
