Вот здесь http://en.wikipedia.org/wiki/P60 определение этой формы на английском языке. Думаю, что выражение "справка об..." удачно, т.к. из налоговой мы часто получаем (или берем) справки о чем-то. Ну, а поскольку речь идет обо всех налогах, уплаченных за год, то моя формулировка, думаю, правильная. Как вариант еще можно сказать "форма Р60" и в скобках дать расшифровку: форма Р60 (справка об общей сумме вычтенных (или уплаченных) за год налогов) |

Обращает на себя внимание, что форма состоит из трех основных позиций: Pay, Tax, NIC (National Insurance Contributions). Поневоле напрашивается аналогия с формой 2-НДФЛ и ее официальным названием.
