  -> It is used along with signatures instead of "printed name." As in "clarification of signature"... I see it all the time. Check the link posted. Right under the line for signature there is a line for "aclaración y título". I could provide many examples.

agree  Mariano Saab: Depende de qué variante del español se trate, no tiene sentido que estén en desacuerdo por cuestiones de uso en cada país. Por ej: en Argentina «letra de molde» no se usa nunca, se usa «aclaración». Por "title" yo usaría «cargo».

nombre en letra de imprenta y cargo

Print name se refiere al nombre en letra de imprenta o aclaración (más común) o letra de molde (menos común).
"Title" se refiere normalmente al cargo en una empresa (falta contexto para saberlo). De lo contrario podría ser simplemente el tratamiento (Sr. Sra. Dr. etc) como dijo Carmen.

  -> Es como dice Mariano más arriba, en el Río de la Plata (Uruguay y Argentina) usamos por lo general aclaración o letra de imprenta, jamás letra de molde (nadie sabría qué quiere decir).
