Accidental overpressure of equipment due to
internal leaks

Accidental overpressure of some components can occur during pressure
testing, normal operation and/or intervention activities. Therefore,
personnel designing/operating/intervening on subsea equipment should
be aware of the potential for components to malfunction (or be incorrectly
operated) and the consequent hazards that can be created due to the
unintentional over pressuring of related components.
By way of example, in some subsea equipment designs, it can be possible
to overpressure particular components, either inadvertently via a leaking
component or due to malfunction of a pressure relief mechanism.
For example:
• some pressure caps used on the tree jumper connection points of a
manifold are not rated to the full working pressure of the production
system and therefore might be inadvertently exposed to a higher
pressure than they are rated for if the -manifold *piping branch*
isolation valve(s)- are opened/leak prior to removal of the cap
