Un guardia real es quien tiene a su cargo la custodia de los miembros de la realeza, y así se lo entiende (no es que sea un guardia de plástico, sino uno de carne y hueso)

Thanks Charles. I googled that too and thought that might be the book in question. But my comment had more to do with the fact that some of the Spanish versions might point in the opposite direction.

If the book in question is the one with this title that comes up when you look, then it is indeed a guardian who is royal, not someone who guards the royals. It's a princess guarding "values".

I believe you can use the word "real" in Spanish without risking ambiguity. If you say "guardia/custodia real" in Spanish nobody would interpret it as "guardia verdadera".
However, it will be interpreted as someone who guards the royals, and not someone from the royalty guarding something or someone else, as I believe it is the case here. For what I can see, Benigno is of the same view, unlike Patinba, who interpreted it the other way round. So this is more relevant than the "royal/real" dilemma methinks.
