English term or phrase: Certificate issued by hospital to certify birth.
Well, I want an English word for a type of certificate. In my country, birth certificate is issued by the authority(official). But in case a child born at the hospital, the hospital will issue this certificate first. Then the parent bring this certificate to their local authority along with other document in order to get the birth certificate.
Even if this don't exist in English, help me name one. Perhaps, birth certifying letter or sth. Please give lots of alternative, so I can choose.

Selected answer:birth notification document / letter
Explanation:
Well, I've just experienced this first hand! ;-)

The midwife who delivers the baby is obliged to inform the authorities of the birth within 36 hours. When you then go to register the birth, they check what you tell them against the information they have from the hospitals. All you have to take in terms of documentary evidence is your discharge notes or your baby's medical notes. So there is no direct equivalent in British English, at least. I don't know how the American system works.

certificate issued by hospital to certify birth.
issued by hospital: Certificate of Live Birth

Explanation:
Hi adda

What US hospitals issue to the parents of each newborn is the "Certificate of Live Birth". Each state has their own standard form that must be completed at the medical cetner where the birth took place.

The Certificate of Live Birth is the piece of paper that is then filed at the Department of Public Health of the pertinent state. The Department of Public Health then issues the ** Certification of Birth Registration**, commonly known as ** Birth Certificate **. This BC is necessary to obtain, for example, a passport for the baby, or to register later the child at school.
