certificate issued by hospital to certify birth. birth notification document / letter Explanation: Well, I've just experienced this first hand! ;-) The midwife who delivers the baby is obliged to inform the authorities of the birth within 36 hours. When you then go to register the birth, they check what you tell them against the information they have from the hospitals. All you have to take in terms of documentary evidence is your discharge notes or your baby's medical notes. So there is no direct equivalent in British English, at least.
