appropriate health care professional acting as a point of contact to provide guidance and support Explanation: I think in your particular context the role of “resource person” can be fulfilled by a variety of appropriate health care professionals designated a point of contact if the patient has any questions or requires guidance and/or support. In your particular sentence, it is being used virtually as a synonym for case manager. Take a look at the following related links. Reduce the intensity of your physical activity or stop it temporarily until your symptoms have improved. Use your action plan (if you have one) and contact your resource person. Contact your doctor and/or healthcare professional if you are unable to resume your physical activities following an aggravation of your COPD symptoms (exacerbation). You may find yourself unable to increase your level of physical activity despite having used the recommendations made in this brochure.
