However, my brochure about leaving with a chronic illness (written in Canada) has it in a more specific context, for example in the following sentence:

Patients who used this program in collaboration with their health professional, “case manager” or ***resource person*** and physician had fewer exacerbations needing hospitalization or emergency room visits.

Any idea what is the role/scope of duties/responsibilities of such person within the Canadian health care system? Obviously, it is not quite the same as case manager.

• Counselling
• Support for people living with and affected by HIV
