"Wie zufrieden sind sie mit der Störungsannahme durch... ...die Service-Leitstelle von ABC bezüglich:"

From the "Service" section of a customer satisfaction survey. (Earlier theories were that "Service" might be "after-sales service", but it seems to be more along the lines of troubleshooting and maintenance.) Those completing the survey have to check on a scale of 1 to 5 how true certain statements are for them (these come after "bezüglich"). I can't find "Störungsannahme" anywhere, apart from a LEO discussion forum. Is it something like "Help hotline"? If it will help, the options after "bezüglich" are: "Erreichbarkeit der Leitstelle", "Freundlichkeit der Mitarbeiter" and "Aufnahme des Störungsverlaufs".

Explanation:
seems to cover the sub-headings that follow. 'Störung' may be redundant given the presence of Service-Leitstelle later in the sentence.

Explanation:
The ...annahme tells me that they are referring to the 'initial' contact with the hotline. Response would tell me that is it the entire trail, from 'annahme' to 'erledigung' (or not).
