anamnesi fisiologica (no English equivalent; see below)


Explanation:
This is a bit complicated, but here we go: I have heard Italians use the word anamnesi fisiologica (so the P could actually be an F), which does not have an equivalent in English. In German, on the other hand, they do have sth called "Vegetative Anamnese", which is basically the same as anamnesi fisiologica, and which does not have an EN eq. either
In En, a complete medical history consists of HPI (history of present illness). PMH (past medical history), SH (social history) and FMH (family history). Usually in reports, allergies are listed separately under Allergies, and weight/height/BMI are listed at the beginning under patient data.

So, I would just break "AP" into 2 parts and say:

ALLERGIES: pollen
PATIENT DATA: weight height BMI
HPI: Low back pain since 2013
etc.
