I am translating a 医療費請求書and 領収書, and a 診療明細書 for the same person.
Both refer to the type of insurance as 自課5.
Does anybody have any ideas? Was this paid privately by the patient?

I have seen things like this used in accounting/billing as shorthand for bill types.

If the date of the receipt is prior to the consumption tax increase, the 課5 to me suggests 課[税]5[%] and the 自 may refer to the taxable amount being the 自己負担 portion.

Just an idea, but hope it helps.
