I prefer "certified by", as its usage seems to be wider, while "authorised" has a rather more severe nuance in English, but either is possible.

In English we put commas in different places.

"I, the undersigned, Maria Popescu, certified translator No. xxx (Ministry....), confirm..... etc"

We never put the surname before the first name. You should never do that.
