Russian term or phrase: тревожные звоночки *in context*
Normally, I would translate тревожные звоночки as warning sign(s), red flag(s), wake-up call(s), but given the context of this sentence, I'm thinking that something more along the lines of "the (hand)writing on the wall" or "the death knell". As in the fatal mistakes that caused the team to lose.

Тревожными звоночками для нашей команды стали ошибки в обороне, которые позволили калужанам создать ряд острых моментов.

Warning sign is also suitable here, just I am thinking "warning bell" is more effective term here in my eyes.

Yes, I saw, I prefer "warning signs" and "warning bells",
warning bells-used to say that something makes someone start to be worried or careful about something

Why don't you use "death knell", I would use this term:
