An Advance Statement (sometimes called a Statement of Wishes) is a general statement about anything that is important to you in relation to your future treatment and wellbeing. You can use it to express your preferences for care or to detail any values or beliefs that inform the decisions you make.

Advance Decision / Living Will

Explanation:
An Advance Decision (also known as a Living Will, Health Care Directive or Personal Directive) is quite different from an advance statement

(see https://www.ageuk.org.uk/globalassets/age-uk/documents/facts...

"An advance decision to refuse treatment lets you, while you have mental
capacity, choose and explain which medical treatments you do not want
doctors to give you, if a time comes when you lack capacity and cannot
make the decision or communicate your wishes. If you want to refuse
life-sustaining treatment, you must put this in writing, sign and date it in
presence of a witness."

It is more than an advance statement which "allows you to make general statements, describing your wishes and preferences about future care should you be unable to make or communicate a decision or express your preferences
at the time."

The term 'Declaración Previa de Voluntad de Deseos Anticipados' is the equivalent of an advance decision (living will) - see https://www.minsalud.gov.co/sites/rid/Lists/BibliotecaDigita...

agree  Taña Dalglish: Statement of Wishes/Living Will (I think, if the option Living Will is used, that takes care of "advance"). Stay safe.

US & UK: Advance Medical Consent Form

Explanation:
a pre-operation form doesn't look to me like a Living Will, an Enduring or Lasting Power of Attorney and consent form works Transatlamtically.

Having drafted consent forms myself, albeit many years ago, albeit I am a bit surprised only David H. has mentioned such in his reference.

Example sentence(s):
  • UK: See also advance directive/living will (eg Jehovah’s Witness form)
  • USA: Protect your child's health when they're in someone else's care. Create a free Child Medical Consent form and authorize a caregiver to make healthcare choices ...
