Source text - English Paralysed woman's thoughts control robotic arm

Unrivalled control of a robotic arm has been achieved using a paralysed woman's thoughts, a US study says.
Jan, who is 53 and paralysed from the neck down, was able to deftly pick up, move and place a variety of objects in a manner similar to a normal arm. Brain implants were used to control the robotic arm, in the study reported in the Lancet medical journal. Experts in the field said it was an "unprecedented performance" and a "remarkable achievement".
Jan was diagnosed with spinocerebellar degeneration 13 years ago and progressively lost control of her body. She is now unable to move her arms or legs. She was implanted with two sensors - each four millimetres by four millimetres - in the motor cortex of her brain. I don't know how to say it any other way, [the movement is] way better than anything that's been demonstrated before” said Prof Andrew Schwartz University of Pittsburgh.
A hundred tiny needles on each sensor pick up the electrical activity from about 200 individual brain cells. „The way that neurons communicate with each other is by how fast they fire pulses, it's a little bit akin to listening to a Geiger counter click, and it's that property that we lock onto," said Professor Andrew Schwartz from the University of Pittsburgh.
The pulses of electricity in the brain are then translated into commands to move the arm, which bends at the elbow, wrist and could grab an object. Jan was able to control the arm after the second day of training and over a period of 14 weeks became increasing skilful. The report said she gained "co-ordination, skill and speed almost similar to that of an able-bodied person" by the end of the study.
Prof Schwartz told the BBC that movements this good had not been achieved before. "They're fluid and they're way better, I don't know how to say it any other way, they're way better than anything that's been demonstrated before. “I think it really is convincing evidence that this technology is going to be therapeutic for spinal cord injured people. "They are doing tasks already that would be beneficial in their daily lives and I think that's fairly conclusive at this point."
The field of harnessing a healthy brain to overcome a damaged body is advancing rapidly. Earlier this year, Cathy Hutchinson used a robotic arm to serve herself a drink for the first time since her stroke 15 years before. In both studies the results were achieved inside a laboratory so are of little help in the home. Researchers are now trying to mount the arm on Jan's wheelchair so she will be able to use it in her everyday life.
There are also attempts to give sensation to the prosthetic arms to restore a sense of touch. In a review researchers Gregoire Courtine, Silvesto Micera, Jack DiGiovanna and Jose del Millan described the control of the arm as "highly intuitive and probably responsible for the unprecedented performance of the brain-machine interface". They added that the system was a "remarkable technological and biomedical achievement" and that such designs were getting closer to a point which "might soon become revolutionary treatment models" for paralysed patients.

Die 53-jährige Jan, die vom Hals an abwärts gelähmt ist, war in der Lage, eine Reihe von Gegenständen geschickt aufzuheben, zu transportieren und anderswo wieder abzulegen, ähnlich wie mit einem normalen Arm. In der Studie, über die in der medizinischen Zeitschrift „Lancet" berichtet wurde, wurden Gehirnimplantate zur Steuerung des Armes verwendet. Führende Neurowissenschaftler sagten, dies sei ein "bisher einmaliger Erfolg" und eine "außergewöhnliche Errungenschaft."
Vor 13 Jahren wurde bei Jan die Diagnose einer spinozerebelläre Ataxie gestellt und seither hat sie immer mehr die Kontrolle über ihren Körper verloren. Inzwischen kann sie ihre Arme und Beine nicht mehr bewegen. Bei der Studie wurden zwei Sensoren von jeweils vier mal vier Millimeter Größe in den motorischen Teil ihrer Großhirnrinde implantiert. "Ich weiß nicht wie ich es anders ausdrücken soll, (die Beweglichkeit ) ist einfach so viel besser als alles, was bisher gezeigt wurde", sagt Professor Andrew Schwartz von der Universität Pittsburgh.
Auf jedem Sensor registrieren einhundert winzige Nadeln die Aktivität von ungefähr 200 einzelnen Gehirnzellen."Die Neuronen kommunizieren miteinander durch die Geschwindigkeit, mit der sie Impulse abgeben. Das ist ungefähr so ähnlich, wie wenn dem Ticken eines Geigerzählers zuhört und genau dieses Phänomen machen wir uns zunutze“, sagt Professor Schwartz.
Die elektrischen Impulse des Gehirns werden in Befehle zur Bewegung des Armes übersetzt. Dieser wird dann im Ellenbogen- oder Handgelenk gebeugt und kann einen Gegenstand ergreifen. Nach dem zweiten Trainingstag konnte Jan den Arm steuern und innerhalb eines Zeitraums von 14 Wochen wurde sie zunehmend geschickter. Im Bericht hieß es, ihre Koordination, Geschicklichkeit und Geschwindigkeit seien a Ende der Studie beinahe mit der eines Gesunden vergleichbar gewesen.
Professor Schwartz sagte gegenüber der BBC, dass eine so gute Beweglichkeit zuvor noch nie erreicht worden sei. „Die Bewegungen sind fließend und so viel besser als alles, was bisher gezeigt worden ist. Anders kann ich es nicht ausdrücken. Meiner Meinung nach ist dies wirklich ein überzeugender Beweis dafür, dass diese Technologie in Zukunft für Rückenmarksgeschädigte von therapeutischem Nutzen sein wird. Die Probanden erledigen jetzt schon Aufgaben, die für sie im Alltag nützlich wären und ich halte das im Moment für ziemlich überzeugend.“
Auf dem Forschungsgebiet, das sich damit befasst, wie ein gesundes Gehirn dazu ausgerüstet werden kann, die Beeinträchtigungen eines geschädigten Körpers zu überwinden, werden zur Zeit rapide Fortschritte erzielt. Anfang dieses Jahres benützte Cathy Hutchison einen Roboterarm, um ohne fremde Hilfe ein Getränk zu sich zu nehmen - zum ersten Mal seit ihrem Schlaganfall vor 15 Jahren. Bei beiden Studien wurden die Ergebnisse innerhalb einer Versuchsanordnung erzielt, können also im Alltag noch nicht umgesetzt werden. Wissenschaftler versuchen nun, den Roboterarm an Jans Rollstuhl zu befestigen, damit sie ihn im Alltag benutzen kann.
Es wird auch versucht, der Armprothese Sensibilität zu verleihen, um so wieder einen Tastsinn herzustellen. In einem Interview sagten die Forscher Gregoire Courtine, Silvesto Micera Jack DiGiovanna und Jose del Millan, die Steuerung des Arms erfolge fast völlig intuitiv. Dies sei vermutlich die Ursache für den bisher einzigartigen Erfolg beim Zusammenwirken von Gehirn und Maschine. Weiterhin sagten sie, das System sei eine außergewöhnliche technologische und biomedizinische Errungenschaft. Solche Apparate könnten vielleicht schon bald zu einer bahnbrechenden Behandlungsmethode für gelähmte Patienten werden.
