Source text - German Zur Komplettierung der Diagnostik führten wir eine Lumbalpunktion durch. Das Liquorlactat war normal. Die Untersuchungen auf Stoffwechselerkrankungen und Neurotransmitter stehen noch aus. Das Schädel-MRT zeigt eine milde Form einer pontecerebellären Hypoplasie, Vermis und Kleinhirnhemisphäre sind dabei gleichmäßig betroffen, insgesamt zeigt sich keine Veränderung zu der vorhergehenden MRT-Untersuchung. Möglicherweise sind die MRT-Veränderungen als sekundär infolge der schweren Epilepsie anzusehen. Alternativ könnte es sich um eine primäre (d.h. genetische) pontocerebelläre Hypoplasie handeln.
Um die Diagnostik zu vervollständigen, wurden die Durchführung einer Muskelbiopsie sowie eine genetische Abklärung angeboten. Diese Untersuchungen wurden von der Familie abgelehnt, die entsprechenden Termine wurden storniert.
Zusammenfassend bleibt daher die Ursache der schweren Behinderung unklar, wir halten entweder eine neurogenetische Störung, als Ursache der "Early myoclonic encephalopathy" oder (trotz normalem Liquorlactat) eine Mitochondriopathie am wahrscheinlichsten.
