Les topiques utilisés sont en effet potentiellement toxiques : corticoïdes, composés aux nombreux effets secondaires locaux, voire systémiques, dérivés de l’hydroquinone aujourd’hui très réglementés en Europe du fait d’un potentiel mutagène au risque incertain, mercuriels, composés à la toxicité bien connue. Malgré des risques établis, ces composés, pour la plupart réservés à l’usage médical, voire même théoriquement bannis comme les mercuriels, sont obtenus à faible prix sur la plupart des marchés africains. Les flux financiers en rapport avec cette pratique semblent importants.
Si les complications médicales de cette pratique apparaissent relativement bien décrites, il en est tout autrement pour ce qui est de ses déterminants. Ce thème, régulièrement abordé dans les médias aussi bien africains qu’occidentaux, sous un angle parfois voyeuriste, connaît des interprétations généralement assez univoques. Selon ces canaux, les utilisatrices de produits dépigmentants sont, de façon quasi exclusive, considérées comme des femmes « cherchant à ressembler aux Blancs », que ce soit par conformisme en quelque sorte « aveugle » vis-à-vis de canons de beauté occidentaux ou, plus profondément, du fait d’une perception dévalorisante de la peau noire. Il en résulte un discours de réprobation morale qui a pu trouver, dans certains pays, un prolongement au travers d’une législation répressive ; en Gambie, cette pratique est ainsi officiellement interdite, les contrevenantes encourant des peines d’amende, voire le renvoi de la fonction publique. Au Sénégal, le décret 79-231 du 9 mars 1979 interdit la pratique du xeesal (nom couramment utilisé pour désigner la dépigmentation cosmétique au Sénégal) aux élèves des établissements d’enseignement élémentaire, moyen et secondaire, sous peine d’exclusion temporaire ou définitive.
Paradoxalement, la recherche en sciences humaines s’est jusqu’à présent peu penchée sur ce phénomène. Quelques données objectives ont pu être obtenues incidemment lors d’enquêtes à objectifs épidémiologiques ou médicaux (Mahé et al., 1993 ; Pitche et al., 1998). Pourtant, selon une optique de recherche fondamentale, mais également dans une logique de santé publique, s’il convient notamment de dissuader les populations de recourir à une pratique aux conséquences sanitaires néfastes et, partant, de promouvoir des actions et messages préventifs dont on voudrait qu’ils aient une certaine efficacité, il paraît indispensable de pousser plus avant l’analyse des déterminants de cette pratique, et cela en se basant sur des données factuelles.
Translation - English The topical compounds used in these treatments are in fact potentially toxic. Corticosteroids are associated with a variety of secondary effects, ranging from local to systemic. Hydroquinone derivatives are currently tightly regulated in Europe due to the unknown risks associated with their mutagenic potential. The toxicity of mercury-containing compounds is widely recognized. In most regions, these compounds are reserved for medical use, or even theoretically outlawed, as in the case of mercury-containing compounds. In spite of these well-established risks, they can nevertheless be obtained at low cost in most African markets. The cash flows generated by the practice appear to be substantial.
While the medical risks associated with skin whitening are relatively well documented, the underlying causes of the practice are less clearly understood. The issue is frequently portrayed, often in a voyeuristic manner, in both African and Western media, where there has in general been a fairly broad consensus regarding its interpretation. According to these sources, skin-whitening products are almost exclusively used by women seeking to emulate the appearance of white women, either as a result of an uncritical desire to conform to Western standards of beauty or, more concerning, due to a perception of black skin as inferior. Such analyses have produced a climate of moral condemnation towards the practice, and in some countries this has extended to the passing of repressive laws. The practice is officially proscribed in The Gambia, for example, where practitioners risk incurring penalties ranging from fines to dismissal from public-sector roles. In Senegal, primary, middle, or secondary-school pupils practising xeesal, the local name for cosmetic skin whitening, face temporary or permanent exclusion from school under decree 79-231 (issued 9 March, 1979).
Surprisingly, the phenomenon has hitherto been largely overlooked by researchers in the social sciences. It has been possible to obtain some objective data from existing medical and epidemiological research (Mahé et al., 1993 ; Pitche et al., 1998). However, if preventative measures aimed at persuading affected populations to abandon this harmful practice are to be effective, it seems necessary from the perspective of both basic research and public health to conduct further research in order to advance our understanding of its underlying causes, and to ensure that this understanding has a sound empirical basis.
