چکیده
زمینه و هدف: آرامبخشی مناسب قسمت مهمی از مراقبت های پرستاری در بخش های ویژه است که در آن با استفاده از یک یا چند دارو باعث کاهش فعالیت سیستم عصبی مرکزی می شوند؛ به طوری که درمان بیمار بدون استرس جسمی و روانی انجام شود. ابزار شاخص دوطیفی یک وسیله ی غیرتهاجمی می باشد که با استفاده از پردازش امواج نوار مغزی می تواند سطح آرامبخشی بیماران را نشان دهد. در این پژوهش به بررسی تأثیر استفاده از سیستم شاخص دوطیفی بر پیامدهای بالینی بیماران تحت تهویه مکانیکی بستری در بخش های ویژه بعد از اعمال جراحی قلب باز پرداختیم.
روش اجرا: در این مطالعه که به صورت کارآزمایی بالینی تصادفی صورت گرفت؛ بعد از کسب رضایت از مسئولین بخش های مراقبت های ویژه مرکز قلب شهید رجایی، 110نفر از بیماران الکتیو کاندید عمل جراحی قلب باز با سن 30-70 سال و کسر جهشی بالای 30 درصد؛ بر اساس معیارهای ورود وارد مطالعه شدند. همه بیماران از یک پروتکل مشابه بیهوشی برخوردار بودند. بعد از عمل جراحی قلب باز بیماران به بخش های مراقبت ویژه وارد شدند. برای بیماران گروه non-BIS پس از ورود به بخش مراقبت ویژه تا زمانی که به ونتیلاتور متصل بودند؛ سطح آرامبخشی طبق روش رایج بخش، براساس نمره مقیاس اغمای گلاسکو و پارامترهای همودینامیک توسط پرستاران سنجیده و ثبت گردید. در گروه BIS از ابتدای ورود بیماران به بخش تا زمانی که به ونتیلاتور متصل بودند؛ سطح آرامبخشی هرساعت یکبار تا 12 ساعت با استفاده از شاخص پایش دوطیفی که به طور مداوم به بیماران متصل بود، در فرم مخصوص خود ثبت گردید. هر دو گروه از پروتکل مشابه آرامبخشی که توسط متخصص بیهوشی تنظیم گردید؛ برخوردار بودند. هر دو گروه وقتی به شرایط قلبی عروقی پایدار رسیدند؛ طبق پروتکل بخش از دستگاه تهویه مکانیکی جدا وسپس اکستیوب گردیدند. داده های جمع آوری شده در نرم افزار SPSS نسخه 22 مورد تجزیه و تحلیل قرار گرفت.
یافته ها: نتایج نشان داد که در گروه BIS کاهش درصد مصرف داروهای آرامبخش و مسکن های اپوئیدی وجود داشت(001.0>P)، ولی در میزان مصرف آپوتل تفاوت معناداری بین دو گروه وجود نداشت(05.0P) ، طول مدت بستری در بخش ویژه(001.0>P) و بیمارستان (001.0>P) به طور معناداری نسبت به گروه BIS کاهش داشت.
نتیجه گیری: استفاده از سیستم شاخص دوطیفی پیامدهای بالینی بیماران تحت تهویه مکانیکی بستری در بخش های ویژه جراحی قلب باز را ارتقا می بخشد. این ابزار می تواند با فراهم نمودن معیاری عینی جهت نشان دادن سطح آرامبخشی، پرستاران بخش های ویژه را در تنظیم دوزاژ داروهای آرامبخش یاری دهد.

Translation - English Background: Proper sedation is the main part of nursing care in the ICU in which using one or more drugs reduces the activity of the central nervous system so that the treatment would be done without physical and mental stress. Bispectral index monitor (BIS) is a non-invasive device that can display the sedation level of patients through EEG processing. Objectives: This study aimed to check if the bispectral index monitor affects the clinical implications of mechanically-ventilated patients in the ICU after the open-heart surgeries.
Materials and Methods: In this randomized clinical trial, 110 patients aged 30-70 years and over 30% Ejection Fraction (EF) were elected as candidates for the open-heart surgery after getting the consent from the authorities of ICUs in Shahid Rajae heart center; they participated in the study based on the inclusion criteria. All the patients had the same anesthesia protocol. After the open-heart surgery, the patients were transferred to ICUs. The sedation level of non-BIS ventilated patients in the ICU was measured and recorded by the nurses based on the Glasgow coma scale and hemodynamic parameters. The sedation level of the given patients since the arrival to the ICU was recorded hourly up to 12 hours through a bispectral index monitor connected to the patients continuously. Both groups had the same sedation protocol adjusted by the Anesthesiologist. They were taken apart from the ventilator (based on the ICU protocol) and then extubated when they reached the sustained cardiovascular conditions. Data were collected and analyzed using SPSS 22.
