Pleural effusion
abnormal collection of fluid between the parietal and visceral layers of pleura.
Radiography in the erect position shows pleural fluid accumulating initially in the lateral and posterior costophrenic angles, then tracking upwards in a meniscus configuration (Fig.1). In smaller children AP radiographs are often obtained in the supine position when fluid gravitates posteriorly. This gives an increase in density to the overlying lung without identification of a discrete effusion. Sometimes fluid may be seen as a pleural "cap" over the apices in the supine position. Pleural effusions may occasionally lie in a subpulmonic position, with little fluid visible elsewhere. On the erect film a subpulmonic collection appears as elevation of the hemidiaphragm, with the dome apparently more lateral in position than normal. On the left, there may appear to be a widening of the space between the stomach bubble and the lung base. A small amount of fluid may track into the pulmonary fissures.

A pleural effusion may also localise as a paraspinal triangular density, more usually on the left in the lower part of the thorax.

Large accumulations of pleural fluid may cause collapse of the underlying lung and mediastinal shift to the opposite side.

Ultrasound confirms the presence of fluid in the pleural space (Fig.2) and guides thoracocentesis. The formation of fibrin strands can be detected, as can loculation which will be helpful in the medical or surgical management. This finding is very suggestive of an underlying bacterial pneumonia with the development of an empyema.

Bacterial pneumonia due to Staphylococcus and Haemophilus influenzae is the commonest cause of pleural effusion in childhood. Effusions are also seen in viral pneumonia and tuberculosis.

Transudate pleural effusions may be seen in cardiac failure in association with pulmonary oedema, renal disease, especially in nephrotic syndrome and collagen vascular diseases, particularly rheumatoid arthritis or systemic lupus erythematosus. Ascitic fluid may enter the chest through the diaphragm.

Other types of fluid seen in the chest include blood, usually secondary to trauma or chyle. See chylothorax

感谢!肺胸科我完全是外行啊,呵呵。。。I picked "cavitation or pleural effusion" because it sounds to me like "空洞和液平"... I didn't quite find any Chinese sources to figure out what exactly is "液平"... thanks again!!
