If it's a document the student is presenting, here we would generally call it a doctor's note to be excused from school for illness.

'Sick leave' only applies to work. A student can have a finite amount of 'sick days' -- a term that may come in handy elsewhere in your translation. For longer periods he/she needs to present proof, i.e. the above certificate.
