If you try the combination polyfarmacie+sanering, you will note that this term is used quite often in situations where too many medications are being taken by a patient. Saneren = 'op orde brengen'; I was not able to find an English equivalent but in reviewing some of the articles I came across, it appears this term means clearing out all of the unecessary meds and keeping the number as low as possible (including stopping or cutting down several meds). This is how I would translate it. Good luck!

This seems to be pretty close to the idea - sorting out what the opatient is taking and making sure the meds are being taken correctly.
