Numero cartella clinica It should have been obvious, but I just didn't see it. The client has kindly pointed out that it stands for "numero cartella clinica". Thank you so much for all your suggestions.

Could it be this? Credit Card number or Carbon Copy number (perhaps copies of the document they're dealing with??

Difficult to say with so little info, but "cc" usually stands for "conto corrente". Now, this would make sense if this was something like a hospital admission forms or such like.
Note from asker:
"Current account" is what sprang to my mind too. I just wasn't sure whether it would fit the context. I think I may have to check this with the client and see if they can shed any light on it.
