Este es el informe médico de un paciente que padece síndrome antifosfolipídico:

El examen físico de ingreso mostró a una paciente obesa, con artritis de rodilla (choque rotuliano más bilateral), edema duro en ambas piernas, lesiones purpúricas de aproximadamente 5 mm infrapatelares, livedo reticularis en el abdomen y miembros inferiores, e hipertensión leve (160-100 mm Hg).

Sé que "choque rotuliano" es "patellar clunk", pero no entiendo a qué se refiere "más bilateral".

Since an arthritis diagnosis, seems to me when he / she mentioned: mas bilateral, refers to: both knees ACUTE.

Some times we classify severity with a plus sign, two, etc... means acute, chronic, etc...

Remember: Choque rotuliano: sensación táctil, y a veces auditiva, que se experimenta empujando bruscamente la rótula contra los cóndilos femorales, cuando existe derrame sinovial...

In the other hand, may be related to classification... See. classification of Shelbourne and / or anatomic classification.

The patellar tap is a test for fluid in the knee joint. This test is likely to be positive with moderate amounts of fluid.

Excess fluid is squeezed out of the suprapatellar pouch with the index finger and the thumb. These are advanced distally from a level about 15 cm above the knee to the level of the upper border of the patella.
