o.k. so It seems to me that the document signed by the judge is for the sole purpose of applying for health coverage
(the document is probably certifying that B is indeed A's domestic partner)

Es una persona la que se va a incorporar a la obra social. La fuente es un certficado firmado por un juez donde A expone que vive en con B en concubinato y desea incorporarla a su obra social (Galeno - prepaga de ARgentina).

@ asker I am assuming that your document is from Argentina where "obra social" is a health plan specific to a guild/union, for example obra social de los empleados del comercio ( http://www.osecac.org.ar/ ). Any person who works and pays taxes belongs to a particular "obra social" .If this is the case then incorporación here would be enrollment and obra social could be union health plan. Could you confirm?

what is being "incorporado"? Could be a person, an asset ... (i.e. hire, add, ...)
Also, is this in the context of a Spanish savings bank, by any chance?

the application to join (the community project)

"to apply for membership" may be a more appropriate translation when we are dealing with an Association. Since this is an "obra social", I would suggest that "joining" is a better fit.

"(the community project)" is tentative. Is this a Charity project? A governmental intervention? Or a community project run by/for the community?
