these are Bank Hapoalim terminology.
It stands for ק"א קבוצת אוכלוסין. תת"ק תאריך תנועה מוקדמת. סו"פ סוג פעולה

Completely agree with Amnon I really believe there's a good chance these are "in-house inventions". The only way to really find out is to contact the bank/the client.

Any context? ק"א could just be "Kiryat Arba" and תת"ק could just be "900". I'm guessing "bank document" is the context as such but I'd need more before even thinking of posting an answer.
