Have Crimean authorities ever issued Ukrainian-language certificates, though — particularly monolingual Ukrainian ones (which seems to be the case with this certificate, as we haven’t been told of a Russian variant on it)?

"Республіканське" may refer to the Crimea (автономна республіка Крим), so I would not translate it automtically as "state", it's better to clarify with the client - in case there is no address on the certificate itself. By the way, state institutions are usually called "державні" or "національні" in Ukraine, so this, too, may indicate that the school in question is not actually a "state" one.
