"failure of measuring probe that sends a signal to the control room" does imply the need for actual instruments. The idea being that an instrument identifies errors, not necessarily the operator. Perhaps Laurent should give us a bit more context.

perhaps two different meanings for "instrumented process" I think "instrument-controlled" is possible, for "documented and instrumented information", perhaps what Ormiston suggests or similar is appropriate?
