To Vladimir Agreed - that's what I was thinking! Still not convinced about "child care facility", but I think it's clear in the context, and is probably intended to cover a wide variety of institutions/organisations. For example, children could be brought in groups from a kindergarten.

@David Exactly. Moreover, it is the attending of this institution by means of which these children happen to be "organized". So if there is any institution (= the children are "organized"), its name is written down. If not (= the children are not organized), the box is left empty. Hence my answer.

To Marina "Regimented children" is not a recognised term in English, and conjures up a picture of children marching like soldiers! The two previous answers (in уже обсуждалось) have the two extremes: "institutionalised children" and "children attending day care centres". Maybe we can't decide. "institutionalised" can mean simply people living in an institution, but tends to mean people who are so used to living in such a place that they can't survive anywhere else (like long-term prisoners). The other version just means children attending a детский сад or something similar and going home at night.

The kids are simply controlled/organized to be brought to the medical institution in question to take a vaccine shot as the statutory rules (a law) prescribe(s).

I propose to think broader and translate the whole field title instead of just the "организованные дети" part. Obviously the whole field is intended to indicate the institution's name if it exists (=if the child is "organized")

agree Elena Ow-Wing : I like this one. Instead of "if applicable" you can say "for attending kids/ for kids who attend one" to avoid ambiguity and make everything 100% clear.

I think this bridges the gap between "childcare" and "institutions", and even retains the word "organised"
