This term is used in some Spanish social services to describe a strategic document used in the different social services departments to plan all the care and support to be provided for a person with disabilities, based on their capacities and their expectations, the aim being to empower individuals by placing them at the centre of the service provision. The document is therefore multidimensional , covering aspects such as employment and training needs, but also health needs, housing needs, social needs and other aspects. At present the translation being used is "life project" but this sounds rather overbearing in English.
Any suggestions?

Thanks to all for your contributions. Each has been useful in its way and I am still discussing possibilities with the client who is unsure now whether the term in Spanish is really appropriate. Uncertain how to give out points when all are useful. Site seems to allow for only one helpful answer, which is odd, given the nature of the work we do. Or maybe I am missing something. Any ideas?

Although Asker hasn't given a quote from the Spanish ST, the explanation suggests this is a what social services professionals (and others) often refer to as a 'road map'. In common with the commonly-misunderstood difference in cartography between a 'plan' and a 'map', a social-worker's 'road-map' it's not as clear-cut, precise, or detailed, as a 'plan'; it does, nonetheless, help those services to work together - and with the beneficiary; and, hopefully, it will give the latter a fair idea of what his/options and/or contraints are for the future.

Thanks, road map is a useful way round the notion of project, which doesnt really work well in english to describe this kind of document. The term is still under discussion with the clients.

life plan/support plan

"Proyecto" also translates as "plan".

I think a literal translation works fine. Alternatively, you could say "support plan".
Example sentence:

life map

Part of the curriculum my wife teaches in secondary school is called "proyecto de vida". She explains it to the kids as a kind of route you draw up to get an overview of how to achieve the goals you set yourself or your proposed lifestyle. I'd never heard the term in English before, but googled "life map" and found this:
