Paciente XXX XXX de 33 años que ingresa el día 05/03/16 con antecedentes de haber sufrido agresión física 12 horas antes siendo traído por servicio de ambulancia, siendo ingresa a la UCI , al momento del ingreso paciente con Glasgow de 8/15 con agitación Psicomotriz y actitud de descorticacion, isocoria pupilar con hiperreactividad de reflejo pupilar izquierda.

I know decortication is a surgical procedure, but how would one have a "decortication attitude"??? Seems to be the wrong word usage somehow.

"decorticate" here refers to brain damage ... see:
