I agree w Germaine, except one detail It's great except that I would stick with "decision" rather than "judgment." Generally a judgment refers to the outcome of an entire dispute (it resolves everything that's disputed, not just one issue). This doesn't sound like a dispute at all, just an administrative process that happens in the court, but to which both sides agree. And this also is just one issue -- the most important one in the legal process of surrogacy, but still, just one.

More fundamentally, a court "order" and a court "judgment" aren't the same thing. This says it is a court order -- in other words, a décision judiciaire or ordonnance d'un tribunal. So "décision" works better than judgment.

Consentement de la gestatrice/mère porteuse intimée à la décision/au jugement

(et non, "intimé(e)" n’est pas réservé aux seuls appels, en tout cas dans les jugements canadiens/québécois)
