El original bulboprotuberancial se refiere a una afectación, en este caso hemorrágica, de ambas estructuras, el bulbo (medulla oblongata) y la protuberancia (pons). La unión bulboprotuberancial se refiere en inglés como pontomedullary junction y al ictus de la región bulboprotuberancial como pontomedullary stroke (ver enlaces más abajo). Curiosamente, sólo he encontrado un ejemplo de hemorrhagic pontomedullary stroke (último link), aunque no parece tan extraño si tenemos en cuenta que la referencia a una afectación "hemorrágica bulboprotuberancial" no encuentra ni un solo ejemplo en la red. Generalmente, tanto en castellano como en inglés, se alude al ACV hemorrágico de esta área bulboprotuberancial como hemorragia bulboprotuberancial (pontomedullary hemorrhage). Así pues, aunque menos literal, tendería a utilizar ésta, pontomedullary hemorrhage, como traducción. Y de sentirte incómodo por la falta de alusión específica a un ACV hemorrágico (hemorrhagic stroke), podrías plantearte expresarlo como hemorrhagic pontomedullary stroke.

Posterior circulation strokes involving the brainstem can result in subsequent ophthalmologic manifestations. Brainstem stroke syndromes are a subtype of strokes which lead to ischemia of the structures of the brainstem. The midbrain, pons, and medulla oblongata are components of the brainstem which control basic body functions such as consciousness, breathing, proprioception, heart rate, and blood pressure. The midbrain—or mesencephalon— and pons have several functions which include involvement in the afferent and efferent pupillary and ocular motor (efferent) pathways. The brainstem including the medulla also contains vestibular-otolith pathways and the ocular sympathetic pathway which can produce skew deviation, nystagmus, or Horner syndrome respectively.

The brain stem comprises the midbrain (attached to the cerebellum), the pons and the medulla (aka the medulla oblongata, at the top of, and connecting with, the spinal cord). These are not separate structures but rather groups of cells that control various distinct functions. The brain stem handles basic functions like breathing, swallowing, heart rate, blood pressure, sleeping and vomiting. The brain stem does not play a part in higher cognitive functions. About 10 percent of all ischemic strokes occur in the brain stem.

The pons is about the size of your thumb; the midbrain is smaller; and the medulla is about the size of the little finger. “All in all, they’re not big structures, but they’re packed full of fibers so even small injuries can hit lots of fibers that are carrying multiple types of information to and from the brain,” Harvey said.
