Just guessing - a doctor mixing up or mixmumbling those complicated letters and/or a typist mis-hearing some letters... SGLT2 inhibitors plus DDP-IV inhibitors would be a plausible combination for diabetes management.
Whatever it is, STWGD is no standard or common abbreviation in medspeak, and you might as well add a translator's note and not worry too much about it.

Note from asker:
Thanks Anne, I agree, anything is possible and guessing would be the only option other than leaving it with a note. |

: Most likely. It is another 'Hemmer' for diabetes also used in German reports.

(standard?) whole-grain diet
I’m fairly sure the WGD stands for “whole-grain diet”. I’m just not sure about the ST part.

Previous studies have reported that whole grain intake alleviated various metabolic syndromes. Here, the hypoglycemic effect of whole grain diet (WGD) on type II diabetes was investigated in C57BL/KsJ-db/db mice.
The above study was in mice, but whole-grain diet is also used in the context of humans:

A Whole-Grain Diet Reduces Peripheral Insulin Resistance and Improves Glucose Kinetics in Obese Adults: A Randomized-Controlled Trial
Purpose
We tested the hypothesis that a WG diet reduces insulin resistance and improves glucose use in individuals at risk for type 2 diabetes compared with an isocaloric-matched refined-grain diet.
