The second input shaft is a low geared unit to apply tension to the rope.
Winding it one-way winds rope on, winding the other winds rope off.
This shaft has a visible ratchet mechanism which engages with the handle and a label “Lift up”

@ Asker: PARSING You need to look at the way you are parsing this: the verb you are looking for here is a trnasitive verb, 'to wind [sth] on / off' — loke cable onto a drum, for example.
