Questionnaire for Swallowing

The maximum tongue pressure at the first examination was 16.3 kPa, but it gradually increased from the first examination to the eleventh, reaching 20.5 kPa at the eleventh examination. The mean value of fatigue VAS during TSE was 15.27 ± 12.71 mm, and the mean value of pain VAS was 0.95 ± 0.45 mm. After eleven treatment visits, the patients were asked to self-answer the EAT-10 and Seirei **Questionnaire for Swallowing**, which showed the scores did not express symptoms of dysphagia.

I could not see **Questionnaire for Swallowing** and saw **Questionnaire of Swallowing** on the Internet. Which do you recommend, of or for?

How many? "the EAT-10 and Seirei **Questionnaire for Swallowing**"
Both of these seem to be questionnaires, so you might be better off using the plural.

"EAT-10: A SWALLOWING SCREENING TOOL"
"Answer each question by writing the number of points in the boxes."

"on" in the sense of "concerning/about"

you could also say "to evaluate" if it fits your context

you could even cut the prepositions out completely and say "swallowing questionnaire"
