I'm entering the answer here because I actually don't agree with the other suggestion, 'diagnosed with SAHT 40 years ago'. The word "diagnóstico" is used ambiguously in Spanish; wwe don't know if it refers to the present or the past. Also "evolución" isn't adding anything helpful to the concept, as Neil noted. There may not be a smooth way to say 'a history of SAHT' in Spanish. Maybe the author used an expression that is more typical in the language. Would it sound natural to say "una historia de SAHT de desde hace 40 años"?
