Swedish term
sjuksköterskemall
Swedish to English
Medical
Medical: Health Care
also läkarmall
Appears in medical records i.e. "sjuksköterskemall grund ögon"
I'm assuming it is a template nurses use for recording results but don't know what the name for this is?
thanks!

nursing report template
Template is certainly the idea, as hinted at by Gullberg SVE/ENG tech. dictionary.
A blank is drawn on this one by the website of the Karolinska Sjukhuset in Stockholm I - in Uni. student halls of residence nearby, all of half a century ago, whilst a Swedish-subsid. contemporary of mine had been summer-jobbing as a 'nursing auxiliary' thereat - use as a go-to reference.
-> 'grund ögon' suggests a one-off record, rather than a report sheet to kick off a (male or female) nurse's shift, as in the first example sentence
Anyway, this is not a query - yet - to refer to a relative of mine, a Prof. Emeritus of Medicine at Glasgow Royal Infirmary, though name-dropping dictates routine mention of him.

Example sentence:
Nursing report sheets are premade templates of paper used by nurses to help them keep track of their patients. A nursing report sheet is started at the beginning of the nurses shift while she/he is getting report from the leaving nurse who is giving them
