محمل قيده في السجل المدني ورقم المسكن

Homework / test Arabic to English Law/Patents Law: Taxation & Customs Somewhat illegible
To me it seems like it should be "مسجل" which would mean registrar, as in the clerk who created and filed the document. But I just want to make sure محمل قيده في السجل المدني doesn't mean something in itself.

But why is ورقم المسك written in the same sentence? Does this mean something different than what I think it does, in this context?
